Photography and video consent & release form

1. Purpose of Use: I consent to the use of my likeness, story, and recordings for educational,

promotional,

and non-commercial purposes. These may include, but are not limited to:

- Social media platforms (Facebook, Instagram, YouTube, etc.)

- The Organization's website and newsletters

- Printed materials, such as flyers or brochures

- Community presentations or events

2. Rights of Use: I understand that these materials may be edited, published, or distributed for the

purposes

mentioned above, and I waive the right to review or approve the final content.

3. No Compensation: I understand that I will not receive financial compensation for my participation

the use of my likeness, recordings, or story.

4. Duration: I grant these rights indefinitely, unless I revoke this consent in writing.

5. Voluntary Participation: My participation in this project is entirely voluntary, and I have the right to

decline or withdraw at any time.

III. Release of Liability

I release and discharge Braving Connection, INC, its board members, officers, employees, and

volunteers from any claims, damages, or liabilities arising from the use of my likeness, recordings,

and story, including any claims of defamation, privacy, or publicity rights.
